Search the Local Offer
Until the age of 18, children with long-term health conditions are supported by children's health and children's social care services. After a child turns 18 years old, they begin to work with adult services. Between the ages of 16 and 18, a young person will start a "transition" of these services:
- health and social care
- mental health
- education
- financial benefits for the young person and their family
- work
- housing
Young people with SEND might need some extra help to thrive and achieve so schools will start planning transition to adulthood with the young person and their family from the age of 14. This will likely involve other agencies like careers guidance organisations.
Some young people with very complex disabilities may be entitled to support from Adult Social Care.
West Berkshire Council's Housing Team will be able to advise on whether young people are entitled to help with housing.
